We ask that your dental practice follow these instructions to help us better serve your laboratory needs:

  • Please fill out a Van Hook Dental Studio lab prescription as completely as possible
  • Place the prescription, models, bites, impressions and/or any other case material(s), secured with foam/bubble wrap to prevent any damage, into a Van Hook Dental Studio shipping box.
  • Tape the box closed and ship via one of the following ways:

 

Local Delivery via IntelliQuick Delivery

Please fill out a Van Hook Dental Studio lab prescription as completely as possible.

Place the prescription, models, bites, impressions and/or any other case material(s), secured with foam or bubble wrap to prevent any damage, into a Van Hook Dental Studio shipping box.

If you are shipping more than one box at a time, please attach the boxes to one another either by taping or rubber-banding them together, or by placing the boxes in a bag

Triple tray impressions, bites and prescriptions for a single case can be sealed in a plastic Ziploc bag. Approximately five (5) bags will fit in one case box.×

Schedule a pickup online or Call Van Hook Dental Studio for pickup: (480) 730-5998. We have two daily pickup times:

  • Schedule by 10:00 a.m. to have your case picked up by noon.
  • Schedule by 2:30 p.m. to have your case picked up by 5:00 PM.

Van Hook Dental Studio will be happy to supply you with all your shipping materials. If you have any questions about your shipping needs or when running low on supplies, please check the appropriate box on your prescription, or contact us.

 

FedEx

Van Hook Dental Studio has a special account with FedEx for shipping cases at reduced rates for our customers. We will provide pre-coded shipping labels for Standard 2-day delivery. Labels for Standard Overnight (for delivery between 11 a.m. and 3 p.m. next business day) and Priority Overnight (for delivery by 10:30 next business day) services are also available upon request.

To schedule pick-up of your cases by FedEx, simply call 1-800-463-3339. At the prompt, state “Customer Service” to be connected to a representative. Or, if you have a regular FedEx delivery driver, he or she will be happy to take cases for you. You can also schedule pick ups or check for the nearest location of a FedEx drop box or facility by visiting FedEx.com and clicking on the “Ship” tab on the home page, then selecting “Find Locations” from the drop-down box. Shipping fees will be charged back to your office according to the delivery service used.

 

United States Postal Service

Van Hook Dental Studio also offers free mail service via the USPS. Simply drop off your case(s) in our pre-printed Van Hook Dental Studio shipping boxes at the Post Office. (Please allow 2-5 days for delivery.)

If you will be shipping more than one box at a time, please tape them together, or place them together in a FedEx bag and use only one air bill.×
